ConnectedNess is a novel Health Information Technology platform designed specifically for the social care referral process, though its data-sharing platform that connects individuals in need to community-based organizations through direct and/or clinician referrals. The platform is composed of three components:

EmpowerNest
Patient designed interface for social needs screening and self-referral management. Available on Android and iOS and supported by a mobile responsive web page. The interface gives agency to the patient by allowing them to conduct the assessment on their phone or in the waiting room. This reduces stigma and increases access to social services because the app is always available. When individuals take the assessment and indicate they have a need, they are immediately presented with organizations that provide those services. Individuals can then choose to connect with those community-based organizations.
CommunityNest
The Community Dashboard powers the mobile app by allowing community-based organizations to register their services. Once a community-based organization is registered, they are allowed to add and modify their services. Real-time updates allow organizations to let everyone know when a particular program is available. Our platform focuses on providing granular real-time updated information. The platform supports large organizations that have multiple staff members working for a specific program. Our goal is to provide a platform that allows the community to curate and share their resources.
EngageNest
Health care teams want to assist and advocate for their patients through this difficult process. EngageNest allows health care teams to be connected with their patients. Now connected health care teams can view the referral status of their patients. Receive status updates and recommend organizations that their patients should connect with.
