How we got here

From clinic pain points to an accountable
community network

We started inside oncology clinics, watching treatment plans slip for reasons outside the EHR—rides, food, housing, bills. Together with navigators and community-based organizations (CBOs), we designed a patient-led way to match needs to local partners and see what happened next. ConnectedNest is that workflow.

Why we started

Treatment was slipping for non-medical reasons. Navigators lacked a simple way to route needs and see outcomes. CBOs couldn’t show capacity or acknowledge quickly.

ConnectedNest—a patient-led workflow that matches needs to curated local partners, under 48 hours acknowledgment, and shares closed loops.
